At Healthcare Connect our goal is to uphold the highest standards of Chronic and Transitional Care Management through education, support, and communication. We value and believe in dignity, inclusion, independence and keeping your information private and confidential.
We provide Chronic Care Management for those with chronic conditions which are expected to last a year or more. Recognized as a crucial part of primary care by the The Centers for Medicare & Medicaid Services (CMS), CCM has shown to promote overall health and reduce healthcare costs.
We also provide Transitional Care Management (TCM) for patients recently discharged from various acute and/or inpatient care facilities. Our goal is to help aid you and your caregiver, whether it’s a family member, nurse, or anyone else manage and maintain the intended trajectory of your health post-discharge.
